Hot-water immersion, thermal bathing and hydrotherapy

What warm water does to the cardiovascular, thermoregulatory, neuromuscular and sleep systems — and what the evidence can and cannot yet claim.

1. What do we mean by “soaking”?

Warm-water immersion is not one single therapy. The physiology depends on water temperature, depth of immersion, duration, the person’s health, and whether treatment includes movement, mineral water, jets or supervised rehabilitation.

Raising body temperature without exercise, using water, sauna or other heating methods.

Thermal stress and the body’s compensatory cardiovascular responses.

This distinction matters because research on a warm bath, a mineral spa treatment, aquatic exercise and sauna exposure cannot automatically be treated as evidence for the same intervention.

2. What happens in the body?

  • The immediate response is well established: the body needs to shed heat while immersion itself changes pressure on the circulation. These effects can feel relaxing, but they are also the reason some people become light-headed when they stand up after a hot soak.

Skin blood vessels dilate

Warmth increases skin blood flow so heat can be transferred from the body core toward the surface. Peripheral vascular resistance usually falls.

Heart rate and cardiac output may rise

Maintaining blood flow to the skin requires cardiovascular adjustment. The response varies with temperature, immersion depth and individual fitness or disease.

Blood pressure can fall transiently

Vasodilation may produce a temporary fall in blood pressure, particularly on leaving the water. The effect is not automatically beneficial for everyone.

Sweating increases as heat load rises

Sweating is a thermoregulatory response. In water, sweat cannot evaporate efficiently from immersed skin, so it contributes less to cooling than it does in air.

Hydrostatic pressure shifts blood centrally

Water pressure around the body can increase venous return and alter stroke volume. This is distinct from the effect of heat itself.

Important distinction
A physiological response is not the same as a proven long-term health benefit. For example, a temporary rise in heart rate does not mean a hot tub provides the same adaptations as aerobic training.

3. What does the evidence support?

Warm bathing is commonly associated with relaxation and may reduce perceived stress, but specific hormone claims are inconsistent and context-dependent.

Moderate for subjective relaxation; low for specific endocrine claims.

Reasonable wellbeing use, but avoid presenting cortisol or endorphin changes as guaranteed.

Sleep

A 2019 systematic review found warm bathing or showering around 1–2 hours before bed could improve sleep efficiency and shorten sleep-onset latency; evidence is not equally strong for all sleep outcomes.

Moderate.

A warm bath can be a useful sleep-hygiene tool, particularly when timed before bed.

Musculoskeletal pain

Balneotherapy/hydrotherapy trials show symptom improvement in some conditions including chronic low-back pain, osteoarthritis and fibromyalgia, although studies are heterogeneous and often combine multiple therapeutic components.

Low–moderate, condition-specific.

Useful as an adjunct for symptom management, not a cure or replacement for diagnosis and rehabilitation.

Cardiovascular / vascular health

Repeated passive heating remains an active research area. Recent RCT meta-analyses find small or inconsistent effects across vascular and cardiometabolic markers; any blood-pressure effect appears modest and heterogeneous.

Low–moderate.

Promising, especially for people unable to exercise fully, but not established as a cardiovascular prevention substitute.

Metabolic health

Recent pooled RCT evidence does not show consistent improvements in glucose, HbA1c, lipids or inflammatory markers across passive-heating studies.

Low–moderate.

Do not claim broad metabolic “reset” effects.

“Detoxification”

Sweat contains trace substances, but there is no good evidence that hot-tub sweating meaningfully removes the body’s toxic burden. Liver, kidneys, lungs and gastrointestinal systems are the principal organs of elimination.

High confidence that “detox” marketing is misleading.

Use hydration and comfort language, not detox claims.

4. Sleep: one of the clearest practical uses

Sleep onset is linked to circadian thermoregulation. A warm bath raises skin temperature and promotes peripheral blood flow; after leaving the bath, heat dissipation can support the normal pre-sleep decline in core temperature. This is a more defensible explanation than saying the bath simply “makes you tired”.

·       A systematic review and meta-analysis found that water-based passive heating at about 40–42.5°C, especially when used 1–2 hours before bedtime, was associated with better self-rated sleep quality and sleep efficiency.

·       As little as 10 minutes of appropriately timed warming was associated with shorter sleep-onset latency in the pooled literature.

·       This does not mean hotter or longer is better. Excess heat, discomfort, dehydration or needing to cool down for too long can be counterproductive.

Practical sleep approach
For a healthy adult, a comfortably warm bath in the evening, completed roughly 1–2 hours before intended sleep, is a reasonable non-drug sleep-hygiene strategy. Hot-tub temperatures should not exceed 40°C. Individual tolerance matters.

5. Muscles, joints and pain

Warm water can reduce perceived stiffness and make movement feel easier. Buoyancy unloads weight-bearing joints, while hydrostatic pressure and warmth can alter sensory input and muscle tone. When exercise is performed in water, resistance allows graded strengthening with lower impact.

For chronic pain, the clinical literature is most persuasive when warm-water treatment is part of a structured hydrotherapy or balneotherapy programme rather than an isolated claim that heat “switches off” pain pathways. Recent reviews report improvements in pain and function in chronic low-back pain, fibromyalgia and osteoarthritis, but study quality, treatment protocols and mineral-water content vary considerably.

What to avoid saying
Heat does not literally “open” joints, repair cartilage, or reliably increase “synovial fluid viscosity” in a way that proves clinical benefit. The safer formulation is that warmth, buoyancy and reduced loading can temporarily improve comfort and range of movement.

6. Cardiovascular effects: promising, but easy to overstate

Hot-water immersion can produce an exercise-like cardiovascular stress in the narrow sense that heart rate, skin blood flow and cardiac output can rise. It does not recruit skeletal muscle, mechanical loading, pulmonary demand or the full metabolic adaptations of exercise. Therefore “exercise mimicry” should be treated as a research analogy, not a health equivalence.

The most up-to-date meta-analyses of repeated passive heating report mixed results. A 2025 review of 20 randomised controlled trials found no significant pooled improvement for most cardiometabolic or vascular outcomes. Systolic blood pressure showed a possible reduction in whole-body heating subgroups, but heterogeneity was substantial. Other 2025 pooled analyses also suggest small blood-pressure effects in some settings, while emphasising variability between protocols and populations.

Bottom line
Passive heating may become a useful adjunct for selected people, including some who cannot exercise normally, but current evidence does not justify describing routine hot-tub use as a proven treatment for cardiovascular disease or as a substitute for physical activity.

7. Stress, mood and the sensory environment

  • Warm immersion can create a strong context for relaxation: reduced external demands, predictable sensory input, muscle comfort and time away from screens or tasks. These factors may support mindfulness and subjective calm. However, claims that soaking reliably lowers cortisol, adrenaline or noradrenaline, or triggers a clinically meaningful endorphin surge, are much less secure than the basic relaxation effect.

For wellbeing content, it is better to describe the experience as a low-demand sensory and physiological relaxation practice rather than assign a single neurochemical mechanism.

8. Skin health: warmth can help comfort, but “detox” is the wrong frame

Warm water increases skin blood flow, but prolonged hot-water exposure can also strip surface lipids, increase transepidermal water loss and aggravate dry or sensitive skin. “Opening pores” is a popular phrase but pores do not open and close like valves. People with eczema or very dry skin may do better with shorter, less hot exposure and moisturising afterwards.

Sweating should not be marketed as detoxification. The body’s principal detoxification and excretion systems are the liver, kidneys, lungs and gastrointestinal tract. Trace chemicals can be detected in sweat, but that is not evidence that hot bathing produces clinically useful removal of toxins.

9. Safe soaking

For most healthy adults, warm-water immersion is straightforward. Risk rises with excessive temperature, prolonged exposure, dehydration, alcohol, sedating medication, cardiovascular instability and poor hot-tub hygiene.

Temperature

Keep hot-tub water at or below 40°C (104°F). More heat is not more therapeutic.

Time

Short sessions are generally easier to tolerate. Around 10–20 minutes is a sensible practical range at hotter temperatures; leave earlier if uncomfortable.

Hydration

Arrive hydrated and replace fluids afterwards. Heat exposure increases fluid loss.

Standing up

Exit slowly. Peripheral vasodilation can contribute to dizziness or fainting, particularly after a hot soak.

Alcohol and sedatives

Avoid alcohol before or during hot-tub use. Sedating medicines and substances may increase impairment, overheating or drowning risk.

Pregnancy

Pregnant people should seek individual advice before hot-tub use, particularly because sustained maternal hyperthermia is undesirable.

Children

Public-health guidance advises that children under 5 should not use hot tubs.

Heart or blood-pressure problems

People with significant cardiovascular disease, arrhythmia, fainting tendency, autonomic dysfunction, or very high/low blood pressure should discuss hot-water exposure with a clinician.

Infection control

Do not use poorly maintained tubs. Appropriate disinfectant, pH control and filtration are essential; contaminated hot tubs can transmit skin, gastrointestinal and respiratory infections including Legionella.

Stop if unwell

Leave immediately for dizziness, nausea, chest pain, breathlessness, palpitations, confusion, severe headache or feeling faint.

10. Evidence at a glance

Well established physiology
Skin vasodilation, altered haemodynamics, increased heat load, hydrostatic-pressure effects and thermoregulatory responses.

Reasonably supported clinical uses
Short-term relaxation; sleep support when warm bathing is appropriately timed; adjunctive symptom relief in selected musculoskeletal pain conditions.

Poorly supported or misleading claims
“Detoxification”; guaranteed hormone changes; “opening pores”; hot tubs as an equivalent replacement for exercise; universal cardiovascular benefit.
